Request An Appointment For Infusion therapy

We strive to make our appointment booking process infusion therapy as simple and stress-free as possible. For Biologic Infusions, while we welcome inquiries at any time, all patients are required to obtain a physician referral for treatment.  

If you are interested in getting treatment at Smart Choice Infusion, you may request a referral from your physician. Your physician can submit a Medication Order form, located in our "Providers" tab.
